JOB DESCRIPTION
· Assess the client’s living environment, determine the individual’s strengths, weaknesses and needs, as well as determine the
strengths and needs of the recipient’s natural support system. Assess the ongoing changes and/or needs and address them.
· Develop a comprehensive services plan in a team approach, which is individualized and contains specific measurable and behavioral
objectives and problems to address the client’s needs. Assist in the treatment team planning and review of the services plan.
· Link the client or family to the community resources. Coordinate and integrate the services provided to the client.
· Monitor the client’s plan of care that includes advocating for the client's needs: Department of Children and Families, and other agencies or organizations.
JOB REQUIREMENTS
· Has documentation of a minimum of a baccalaureate degree from an accredited university, with major course work in the areas of
psychology, social work, health education or a related human services field and has a minimum of one year of full time or equivalent
experience working with clients with special needs.
· Has completed AHCA approved case management training (attach Case Manager Training Certificate) and will complete periodic
retraining, as required by the district.
· Knowledge of all available resources in the service area for client with mental illness.
· Knowledgeable of and will comply with state and federal statutes, rules and policies that effect the target population.
